RESUMO

It's reported that 10% of the inpatients by traumas have fractured ribs. It's easy to judge if it's the case of flail chest, or respiratory failure due to airway injury or pulmonary contusion. There is no guideline for treatments of a patient who has multiple ribs fractures without respiratory failure. We did the surgical stabilization of rib fractures for 10 cases from 2020 to October 2023. As a result, we find out useful surgical indication for treatment of rib fractures.

Several studies suggest that leisure activities enhance well-being. In line with this perspective, a recent study indicates that augmenting indoor leisure activities to compensate for diminished outdoor pursuits could sustain or enhance well-being during the COVID-19 pandemic. The present study was designed to identify personality traits that predict such behavioral shifts in indoor versus outdoor leisure activities during the pandemic. The present study included 657 participants (Mage = 41.08) and measured 12 personality traits that a previous study reported were associated with health-protective behaviors during COVID-19. Our findings indicate that the rise in indoor leisure activities correlated with prosocial tendencies toward family and friends/acquaintances (but not strangers), self-centered interest, resilience, and Big Five personality traits. Conversely, the decline in outdoor activities was linked solely to prosociality toward family and friends/acquaintances. Further interaction analysis uncovered that prosocial tendencies toward close relations predicted increased indoor activities as an alternative to outdoor engagements. We concluded that prosociality promoted behavioral changes that significantly prevented infections in intimate others, and it could maintain personal well-being during the COVID-19 pandemic by facilitating behavior change.

(1) Background: Inflammatory responses induce the formation of both anti-tumor and pro-tumor neutrophils known as myeloid-derived suppressor cells (MDSCs). Intermittent intravesical infusion of Bacillus Calmette-Guérin (BCG) is an established cancer immunotherapy for non-muscle-invasive bladder cancer (NMIBC). However, the types of neutrophils induced via the inflammatory response to both tumor-bearing and BCG remain unclear. (2) Methods: We therefore analyzed neutrophil dynamics in the peripheral blood and urine of patients with NMIBC who received BCG therapy. Further, we analyzed the effects of BCG in a mouse intraperitoneal tumor model. (3) Results: BCG therapy induced the formation of CXCL10 and MHC class II-positive neutrophils in the urine of patients with NMIBC but did not induce MDSC formation. CXCL10- and MHC class II-expressing neutrophils were detected in peritoneal exudate cells formed after BCG administration. Partial neutrophil depletion using an anti-Ly6G antibody suppressed the upregulation of CXCL10 and MHC class II in neutrophils and reversed the anti-tumor activity of BCG in mouse models. (4) Conclusions: These results indicated that intracellular MHC class II- and CXCL10-expressing neutrophils indicate the state of anti-tumor activity induced via BCG. The status of neutrophils in mixed inflammation of immunosuppressive and anti-tumor responses may therefore be useful for evaluating immunological systemic conditions.

The enforcement of nationwide lockdowns and social distancing measures severely restricted behavior and led to increases in stress, anxiety, and depression during the COVID-19 Pandemic. However, contrary to expectations, studies show that well-being did not decrease significantly during the Pandemic. The present study examined whether intentional increases in alternative behaviors contributed to maintaining well-being. We predicted an increase in indoor activities as alternatives to outdoor activities and that these behavioral changes contribute to maintaining well-being. Focusing on leisure activities, transport mode, and working environments, we tested these predictions in an online survey of 1,000 participants (M = 40.4 years; SD = 10.9). The results demonstrated that the decrease in outdoor leisure activities (e.g., traveling and shopping), use of public transportation, and working at office led to a reduction in well-being. It was also demonstrated that the subsequent increase in indoor leisure activities (e.g., exercising at home and online shopping) and use of a private car led to an increase in well-being, which supported our predictions. These results suggest that increasing alternative behaviors can maintain overall well-being during pandemics. These findings highlight the significance of intentional behavioral changes in maintaining well-being during pandemics.

RESUMO

A 28-year-old male presented to our hospital with hemoptysis and his chest computerized tomography (CT) showed the right middle and lower lobe atelectasis due to the tumor of right intermediate bronchial trunk. To reduce the blood flow to the tumor, bronchial arterial embolization was performed and the tumor was resected using Cryoprobe with a flexible endobronchial scope. Thus, we could observe the tumor localization and diagnose before the surgical procedure. We performed the right sleeve middle lobectomy and the right lower lobe was safely preserved.

The therapeutic outcome of immune checkpoint inhibition (ICI) can be improved through combination treatments with ICI therapy. Myeloid-derived suppressor cells (MDSCs) strongly suppress tumor immunity. MDSCs are a heterogeneous cell population, originating from the unusual differentiation of neutrophils/monocytes induced by environmental factors such as inflammation. The myeloid cell population consists of an indistinguishable mixture of various types of MDSCs and activated neutrophils/monocytes. In this study, we investigated whether the clinical outcomes of ICI therapy could be predicted by estimating the status of the myeloid cells, including MDSCs. Several MDSC indexes, such as glycosylphosphatidylinositol-anchored 80 kD protein (GPI-80), CD16, and latency-associated peptide-1 (LAP-1; transforming growth factor-ß1 precursor), were analyzed via flow cytometry using peripheral blood derived from patients with advanced renal cell carcinoma (n = 51) immediately before and during the therapy. Elevated CD16 and LAP-1 expressions after the first treatment were associated with a poor response to ICI therapy. Immediately before ICI therapy, GPI-80 expression in neutrophils was significantly higher in patients with a complete response than in those with disease progression. This is the first study to demonstrate a relationship between the status of the myeloid cells during the initial phase of ICI therapy and clinical outcomes.

BACKGROUND: The regulation of the redox balance in the tumor microenvironment is thought to be an adaptive response of tumor cells to hypoxic environments. In recent years, it has been reported that the hemoglobin ß-chain (HBB), which is involved in scavenging reactive oxygen species (ROS), is expressed in several carcinomas. However, the relationship between HBB expression and the prognosis of renal cell carcinoma (RCC) remains unclear. METHODS: HBB expression was immunohistochemically analyzed in 203 nonmetastatic clear cell RCC (ccRCC) cases. Cell proliferation, invasion, and ROS production were measured in ccRCC cell lines treated with HBB-specific siRNA. RESULTS: The prognosis of HBB-positive patients was worse than that of HBB-negative patients. Cell proliferation and invasion were inhibited, and ROS production was increased by treatment with HBB-specific siRNA. Oxidative stress increased HBB expression in cells exposed to H2O2. CONCLUSIONS: HBB expression in ccRCC contributes to cancer cell proliferation by suppressing ROS production under hypoxic conditions. Taken together with clinical results and in vitro experiments, HBB expression may serve as a new prognostic biomarker for RCC in the future.

Derivatives of the Chinese traditional medicine indirubin have shown potential for the treatment of cancer through a range of mechanisms. This study investigates the impact of 6'-bromoindirubin-3'-acetoxime (BiA) on immunosuppressive mechanisms in glioblastoma (GBM) and evaluates the efficacy of a BiA nanoparticle formulation, PPRX-1701, in immunocompetent mouse GBM models. Transcriptomic studies reveal that BiA downregulates immune-related genes, including indoleamine 2,3-dioxygenase 1 (IDO1), a critical enzyme in the tryptophan-kynurenine-aryl hydrocarbon receptor (Trp-Kyn-AhR) immunosuppressive pathway in tumor cells. BiA blocks interferon-γ (IFNγ)-induced IDO1 protein expression in vitro and enhances T cell-mediated tumor cell killing in GBM stem-like cell co-culture models. PPRX-1701 reaches intracranial murine GBM and significantly improves survival in immunocompetent GBM models in vivo. Our results indicate that BiA improves survival in murine GBM models via effects on important immunotherapeutic targets in GBM and that it can be delivered efficiently via PPRX-1701, a nanoparticle injectable formulation of BiA.

A previous study reported that reaction times (RTs) and the amplitude of the P1 component of event-related potentials (ERPs) elicited by visual stimuli decreased during visuospatial implicit sequence learning in the serial reaction time task, suggesting that sequence learning reduces attentional demands on visual stimulus processing. In the present study, to evaluate the replicability of the previous finding and to obtain a better understanding of how visual stimulus processing is affected by visuospatial implicit sequence learning, we measured ERPs and neural synchrony from 44 participants during a modified serial reaction time task which controlled for a possible confounding factor in the previous study (i.e., arousal). The results indicated that RTs and neural synchrony of the lower frequency band (22-34 Hz) decreased for a learned sequence, whereas no significant effects on the amplitudes of P1, N1, and P3 components of ERPs were observed. These results suggest that attentional demands on visual stimulus processing can be reduced by visuospatial implicit sequence learning, as suggested by the previous study, but stimulus-locked ERPs may not be sensitive enough to reflect such learning effects.

Ectopic excitability in pulmonary veins (PVs) is the major cause of atrial fibrillation. We previously reported that the inositol trisphosphate receptor in rat PV cardiomyocytes cooperates with the Na+-Ca2+ exchanger to provoke ectopic automaticity in response to norepinephrine. Here, we focused on adenylyl cyclase (AC) as another effector of norepinephrine stimulation. RT-PCR, immunohistochemistry, and Western blotting revealed that the abundant expression of Ca2+-stimulable AC3 was restricted to the supraventricular area, including the PVs. All the other AC isotypes hardly displayed any region-specific expressions. Immunostaining of isolated cardiomyocytes showed an enriched expression of AC3 along the t-tubules in PV myocytes. The cAMP-dependent response of L-type Ca2+ currents in the PV and LA cells is strengthened by the 0.1 mM intracellular Ca2+ condition, unlike in the ventricular cells. The norepinephrine-induced automaticity of PV cardiomyocytes was reversibly suppressed by 100 µM SQ22536, an adenine-like AC inhibitor. These findings suggest that the specific expression of AC3 along t-tubules may contribute to arrhythmogenic automaticity in rat PV cardiomyocytes.

Event-related brain potentials (ERPs) elicited by auditory stimuli unrelated to a current visual-cognitive task (i.e., task-irrelevant auditory probes) can be used to evaluate the level of mental workload. Towards the evaluation of workload in the shortest possible time, the present study with a multiple-stimulus paradigm (Takeda and Kimura, 2014, Int. J. Psychophysiol.) examined whether manipulating time intervals between probes could improve the temporal resolution in evaluating workload. Probes were presented in four interval conditions as a combination of two mean interval lengths [long (600 ms) vs. short (300 ms)] and two interval variabilities [variable (five levels) vs. fixed], while participants were performing a driving game at slow and fast speeds (i.e., imposing low and high workload, respectively). For each interval condition, the minimum data length required to obtain a significant difference in the amplitude of ERPs (i.e., auditory N1 and P2) between the slow and fast driving tasks was estimated. The N1 difference was significant in all four interval conditions but the required minimum data lengths to observe this difference did not greatly differ across the interval conditions (about 60-90 s). The P2 difference was significant only in the long-variable condition and the required minimum data length was about 120 s. These results suggest that, at least with a multiple-stimulus paradigm, manipulations of time intervals between probes did not greatly improve the temporal resolution in evaluating mental workload; at present, long-variable intervals would be optimal for evaluating mental workload in detail.

BACKGROUND: Motorized transport access and out-of-home activities are two potential correlates of Life-space mobility (LSM), a common research topic in mobility studies of older adults. These correlates remain mostly unexplored in previous literature and relating them with LSM can reveal directions for improving the LSM of older adults. METHODS: The associations between motorized transport access, out-of-home activities, and LSM were examined using data from 1,333 older adults (mean age = 70.63) living in 15 cities and towns in Japan. LSM was assessed using composite life-space assessment (LSA) scores. Motorized transport access was measured using dummies showing whether a person had car access (divided into five levels) and used public transport (bus and railway), and out-of-home activities were measured using the number of various activities that were conducted during the most recent weekday and weekend day. Generalized linear models were used to assess the associations. RESULTS: The sample was dominated by males (74.42%), with more than half of the sample had their own cars. On average, each respondent had four activities during two survey days, and shopping was the most common activity. The results showed that owning a car and using railway, as well as various activities were associated with increased composite LSA scores, whereas no cars or only shared cars in home were associated with decreased composite LSA scores. However, these associations differed between males and females. CONCLUSIONS: In this study, different levels of motorized transport access and different types of out-of-home activities were found to associate differently with composite LSA scores. Based on these findings, we suggest that policymakers should provide more transport access, pay more attention to the LSM of older adults with high clinics/hospital activities, and trigger more shopping and daily leisure activities for older adults to improve the LSM of this population.

Recent studies have discovered a relationship between glycosylphosphatidylinositol (GPI)-anchored protein 80 (GPI-80)/VNN2 (80 kDa GPI-anchored protein) and malignant tumors. GPI-80 is known to regulate neutrophil adhesion; however, the action of GPI-80 on tumors is still obscure. In this study, although the expression of GPI-80 mRNA was detectable in several tumor cell lines, the levels of GPI-80 protein were significantly lower than that in neutrophils. To clarify the function of GPI-80 in tumor cells, GPI-80-expressing cells and GPI-80/VNN2 gene-deleted cells were established using PC3 prostate cancer cells. In GPI-80-expressing cells, GPI-80 was mainly detected in vesicles. Furthermore, soluble GPI-80 in the conditioned medium was associated with the exosome marker CD63 and was also detected in the plasma obtained from prostate cancer patients. Unexpectedly, cell adhesion and migration of GPI-80-expressing PC3 cells were not modulated by anti-GPI-80 antibody treatment. However, similar to the GPI-80 family molecule, VNN1, the pantetheinase activity and oxidative state were augmented in GPI-80-expressing cells. GPI-80-expressing cells facilitated non-adhesive proliferation, slow cell proliferation, NF-κB activation and IL-1ß production. These phenomena are known to be induced by physiological elevation of the oxidative state. Thus, these observations indicated that GPI-80 affects various tumor responses related to oxidation.

Event-related potentials elicited by tones generated by one's own discrete actions (e.g., button presses) are attenuated compared to those elicited by tones generated externally. The present study investigated whether ERP attenuation would occur when the timing or pitch of tones is modulated by continuous actions, as for such actions, a weak association between actions and their auditory consequences is assumed. In a modulation condition, participants modulated the time interval between tones (Experiment 1) or the pitch of tones (Experiment 2) by turning a steering wheel. In a listening condition, participants listened to the same tones as in the modulation condition without any action. The results revealed that the amplitude of N2 elicited by tones decreased in the modulation compared to listening conditions, consistently in the two experiments, suggesting relatively higher-order auditory processing can be mainly influenced by the prediction of action consequences when continuous actions modulate features of auditory stimuli.

Previous studies have proposed that holding soft objects can increase expectations to be included in a Cyberball task. The present study investigated whether effects of holding soft objects on expectations are restricted to social contexts or can appear in non-social contexts. Sixty-six participants performed a social or non-social ball task, both modified versions of the Cyberball task. In the social ball task, participants were told that they would play a ball-tossing game with computer-generated players. In the non-social ball task, participants were told that they would take part in a judgement task. During the task, participants held either soft or hard cushions, and their electroencephalographic signals were recorded to evaluate the contingent negative variation (CNV), which is considered to reflect expectation and anticipation of an imperative stimulus. The results showed that participants who performed the social ball task exhibited larger late CNV when they held the soft cushion compared to when they held the hard cushion, whereas participants who performed the non-social ball task exhibited no such difference. The results indicate that holding soft objects increases expectations to be included only in social contexts.

RESUMO

A 74-year-old man had previously been treated for primary malignant melanoma of the bladder. Three years after surgery, 8 mm round nodule in the right lower lobe was found on chest computed tomography. The nodule increased to 11 mm after three months, and resected with a diagnosis of metastatic malignant melanoma. By pathology, it was diagnosed as an intrapulmonary hematoma.

Many studies have reported attentional biases based on feature-reward associations. However, the effects of location-reward associations on attentional selection remain less well-understood. Unlike feature cases, a previous study that induced participants' awareness of the location-reward association by instructing them to look for a high-reward location has suggested the critical role of goal-driven manipulations in such associations. In this study, we investigated whether the reward effect occurred without goal-driven manipulations if participants were spontaneously aware of the location-reward association. We conducted three experiments using a visual search task that included four circles where participants received rewards; one possible target location was associated with a high reward, and another with a low reward. In Experiment 1, the target was presented among distractors, and participants had to search for the target. The results showed a faster reaction time in the high-reward rather than the low-reward locations only in participants aware of the location-reward association, even if they were not required to look for the association. Moreover, in Experiment 2, we replicated the main findings of Experiment 1, even when the target had an abrupt visual onset to restrict goal-driven manipulations. Furthermore, Experiment 3 confirmed that the effect observed in Experiment 2 could not be attributed to the initial eye position. These findings suggest that goal-driven manipulations are unnecessary for inducing reward biases to high-reward locations. We concluded that awareness of the association rather than goal-driven manipulations is crucial for the location-reward effect.

Aldehyde reductase encoded by the Akr1a gene catalyzes the NADPH-dependent reduction of a variety of aldehyde compounds, and it plays a role in the biosynthesis of ascorbic acid (AsA) by converting D-glucuronate to L-gulonate. Although supplementing drinking water with AsA (1.5 mg/mL) ameliorates the fertility of Akr1a-/- (KO) female mice, litter sizes in the KO mice are typically smaller than those for Akr1a+/+ (WT) mice, and about one-third of the neonates have a reduced stature. Half of the neonates in the smallest, developmentally retarded group died before weaning, and the remaining half (less than 6 g in weight) also barely grew to adulthood. While no difference was found in the number of fetuses between the KO and WT mice at 14.5-embryonic days, the sizes of the KO fetuses had already diverged. Among the organs of these retarded KO neonates at 30 d, the spleen and thymus were characteristically small. While an examination of spleen cells showed the normal proportion of immune cells, apoptotic cell death was increased in the thymus, which would lead to thymic atrophy in the retarded KO neonates. Plasma AsA levels were lower in the small neonates despite the fact that their mothers had received sufficient AsA supplementation, and the corticosterone levels were inversely higher compared to wild-type mice. Thus, insufficient AsA contents together with a defect in corticosterone metabolism might be the cause of the retarded growth of the AKR1A-deficient mice embryos and neonates.

A previous study suggested that holding soft objects enhanced expectations of uncertain events and increased social pain under frequent negative feedback; i.e., higher expectations might have induced more disappointment. The present study examined the effects of holding a soft cushion under frequent positive feedback. Participants (n = 42) performed fair-play and over-inclusion blocks in the Cyberball task. Amplitudes of the contingent negative variation (CNV) of event-related brain potentials and subjective ratings of social pain were measured to estimate participants' expectations and emotions, respectively. CNV amplitudes were higher in the over-inclusion block when participants held the soft than the hard cushion. There was a statistically marginal trend (p = .095) for lower social pain scores in the soft cushion condition than the hard cushion condition in contrast to previous findings. These results suggest that holding a soft object does not directly modulate emotions but instead acts through the mediation of enhanced expectations.

Granulocyte and monocyte adsorptive apheresis (GMA), an effective therapy for inflammatory disorders, exerts an anti-inflammatory influence by utilizing the biological reaction between blood and cellulose acetate (CA) beads, which are the carriers of the GMA column. Although the biological reaction has an optimum temperature, blood contacts the CA beads below body temperature as GMA is performed in an extracorporeal circulation system. We investigated various soluble factors in blood treated with CA beads at 25°C and 37°C. Here, the optimal temperature for IL-1 receptor antagonist (IL-1ra) release induced by CA beads was 37°C, and IL-6 production from monocytic cells was inhibited by the addition of plasma prepared from the CA bead-treated blood at 37°C, rather than at 25°C. These results indicated that physiological heating of the apheresis carrier augmented the anti-inflammatory reaction in vitro. Thus, heating during GMA may be a new approach for augmenting clinical efficacy.
